Identification and vital records are often needed for housing, benefits, employment, health care, banking, and other services. HTHS may be able to help you understand the process, gather requirements, use online systems, print forms, and connect with the correct issuing agency.
Protect your information: Do not place a Social Security number, full birth date, document number, medical details, password, or photograph of an identity document in public feedback or ordinary online notes. Share sensitive information only with authorized staff when it is necessary.
Documents We May Help With
- Kentucky identification cards or driver credentials.

What to Bring, If Available
- Old, expired, damaged, or photocopied documents.

- Payment information only when an official agency requires a fee.
