HTHS may be able to help veterans, eligible family members, and authorized representatives request military service records such as a DD Form 214 or other separation and personnel records.
Privacy matters: Military record requests may require a Social Security number, service number, signature, dates of service, and proof of relationship or authorization. Do not place this information in public feedback or ordinary online notes.

Information to Gather
- The veteran's complete name used during service.
- Branch of service and approximate service dates.
- Date and place of birth.
- Service number or Social Security number, when required.
- The specific document or information needed and the reason for the request.
- Proof of next-of-kin relationship or written authorization, when applicable.

Records are not normally produced immediately. The National Archives controls access, required proof, processing, and delivery. HTHS cannot guarantee that a record exists, that a requester is eligible to receive it, or when the request will be completed.
